VOLLEYBALL CALGARY RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS, AND INDEMNITY AGREEMENT
BY COMPLETING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY.

Group Organizer’s Name: Volleyball Calgary ("VC")
 

ASSUMPTION OF RISKS

I acknowledge that participating in sports activities associated with VC, including those organized by its affiliated programs, involves inherent risks. I understand that by allowing my child to participate, I am freely accepting and fully assuming all associated risks, dangers, and hazards, including the possibility of personal injury, death, or property damage.
 

RELEASE OF LIABILITY, WAIVER OF CLAIMS & INDEMNITY AGREEMENT

In consideration of VC and its affiliated programs providing my child with the opportunity to participate in activities and use the facilities and equipment, I agree to the following:

1. TO WAIVE ANY AND ALL CLAIMS that I may have now or in the future against VC and its affiliated programs, including their directors, officers, employees, agents, representatives, and successors.

2. TO RELEASE VC AND ITS AFFILIATED PROGRAMS from any liability for loss, damage, injury, or expenses my child or I may suffer as a result of participating in activities, regardless of the cause, including NEGLIGENCE, BREACH OF CONTRACT, BREACH OF STATUTORY DUTY OF CARE, OR OCCUPIERS' LIABILITY on the part of VC and its representatives.

3. TO HOLD HARMLESS AND INDEMNIFY VC and its affiliates from any liability for property damage or personal injury to third parties resulting from my child’s participation in activities associated with VC.

4. BINDING AGREEMENT: I acknowledge that this Agreement shall apply to and be binding on my heirs, next of kin, executors, administrators, and assigns in the event of my death.

I have read and understand this Agreement. I am aware that by clicking "I Accept" on the VC online registration form or submitting a printed form, I am waiving certain legal rights on behalf of myself, my child, and my legal representatives.
 

PERSONAL INFORMATION CONSENT

I consent to VC collecting, using, and disclosing personal information about my child, myself, and other family members solely for administering VC programs.
 

PHOTO AND VIDEO AUTHORIZATION

I authorize VC and its affiliated programs to photograph or record video of my child or myself during activities for promotional purposes, including online and print media, without charge.
 

FILMING RESTRICTIONS

I understand that no filming of athletes is permitted during Volleyball Calgary training sessions unless conducted by an authorized videographer. Parents who violate this policy may have their child removed from the program.
